IMMUNIZATION REQUIREMENTS

The Texas Administrative Code mandates that “every child in the state must be immunized against vaccine preventable diseases caused by infectious agents.” (TAC RULE §97.63) A comprehensive list of the required vaccinations can be obtained from our school nurse. Should a student fail to meet these requirements, they will NOT be permitted to attend school (TAC RULE §97.66). Tanglewood Middle School will inform students in writing if they are not in compliance and will make every effort to follow up with a phone call to the parent or guardian. Please do NOT disregard this notice from the school nurse. It is your responsibility and obligation to ensure that the school has your up-to-date address and phone number on record.

The following immunizations are required:

•    DTaP/DT – four or five (4 -5) doses required with last dose after age 4 and 1 within 10 years. 
      Tdap Booster - For 7th grade: one (1) dose of Tdap is required if at least five years have passed since the last dose of tetanus-containing vaccine.
•    Polio (oral) four (4) doses required, with last dose after age 4. 
•    MMR (rubella, rubeola, mumps) 2 doses required: 1st dose received on or after the first birthday. 
•    Hepatitis B – three (3) doses required. 
•    Hepatitis A – two (2) doses required. with the first dose received on or after the first birthday.
•    Varicella (Chickenpox) two (2) doses required, 1st dose on or after the 1st birthday (or history of illness). 
•    Meningococcal (MCV4) – one (1) Dose (For 7th and 8th Grade Students, on or after age 11)
 

TEMPORARY OR ONGOING MEDICAL CONDITIONS

If your child is facing a temporary or ongoing medical condition that may (a) necessitate medical attention at school, and/or (b) impact their ability to thrive in the educational environment, it is crucial for the nurse to be notified. Please submit a written diagnosis from a licensed physician on official HISD forms that confirm your child's medical status. Please make sure that both the school nurse and your child's teacher/principal are aware of the situation, enabling us to effectively meet your child's needs. Additionally, HISD provides resources that may be advantageous to you. You can be assured that personal medical information will always be kept confidential.

ADMINISTERING MEDICATIONS AT SCHOOL

School Board Policy (as per TEC §22.052) prohibits nurses and teachers from administering any medications at school without the necessary consent. A student is NOT allowed to carry medication with him/her, except for those diagnosed with Asthma who have submitted a written order from a licensed physician or health care provider. All medications must be secured in the Nurse’s Clinic. A written medication permission form, which must include the original signatures of both a parent and the prescribing medical health care provider, are required for all medications. This includes those for self-administration of asthma prescriptions. Each medication must remain in its original container, complete with a prescription label that identifies the patient for whom the medication was prescribed to, the name of the drug, and the correct dosage and administration route information. These regulations apply to ALL medications, encompassing both prescription and over-the-counter drugs. Medication and Treatment Forms are available from the Nurse’s Clinic.
